The effects of using parecoxib sodium for analgesia on post-hepatectomy cognitive function in elderly patients were investigated. Eighty patients over 60 years of age undergoing elective hepatectomy for liver cancer were randomly assigned into observational group and control group, with 40 cases in each group. Patients in observational group received parecoxib sodium administration after surgery (40 mg, i.v. injection, and once per 12 h for 3 d), and those in control group were given normal saline as placebo. And addnl. tramadol was used in either group according to need for analgesia to maintain the pain visual analog scale (VAS) not more than a score of 3. The Wechsler Adult Intelligence Scale (WAIS) and Wechsler Memory Scale (WMS) were used to test the incidence of postoperative cognitive dysfunction (POCD) in both groups of patients, and the blood levels of S-100β protein, inflammatory cytokines, cortisol, and ammonia were detected before surgery and on several time points after surgery. The perioperative data between the two groups of patients showed no statistical difference. The incidence of POCD was significantly lower in observational group than that in control group (7.5% vs. 2.5%, P<0.05). The levels of cortisol, interleukin-4 (IL-4) and ammonia had no significant alteration in both group at any postoperative time points compared with their preoperative levels. The levels of S-100β protein, IL-6, tumor necrosis factor-α (TNF-α), IL-1β and C-reactive protein (CRP) in both groups were significantly increased in different time spans with varying degrees in both groups compared with their preoperative levels, but the increasing degrees of S-100β protein and IL-6 in observational group were less than those in control group within postoperative 24 and 48 h, resp. Parecoxib sodium administration could help improve the post-hepatectomy cognitive function in elderly patients, and the mechanism might probably associated with its decreasing the S-100β and IL-6 levels.

Objective: To compare the effect of parecoxib sodium and tramadol in combination with propofol on postoperative analgesia in painless induced abortion. Methods: 120 Cases of pregnant women of ASA I-II grade undergoing painless induced abortion were randomly divided into three groups. P group was preoperatively injected with 40 mg of parecoxib sodium, Q group was preoperatively injected with 100 mg of tramadol, and X group was preoperatively injected with 40 mg of parecoxib sodium and 100 mg of tramadol, and all groups were treated with 1.5-2.5 mg/kg propofol for anesthesia. The intraoperative consumption of propofol, intraoperative and postoperative analgesic effect and adverse reactions in the three groups were compared. Results: The intraoperative consumption of propofol in X group was significantly lower than that in P group and Q group (P < 0.05). All groups have good intraoperative analgesic effect without statistically significant difference (P > 0.05). The uterine contraction pain VAS scores at each time point during surgery in X group were significantly lower than those in P group and Q group, with statistically significant differences (P < 0.05). The difference of incidence of nausea and vomiting between the three groups was not statistically significant (P > 0.05). Conclusion: Application of parecoxib sodium combined with tramadol in painless induced abortion can reduce the intraoperative consumption of propofol, and has better efficacy than simple parecoxib sodium or tramadol.

The effects of parecoxib Na on inflammatory mediator and postoperative cognitive dysfunction (POCD) after laparoscopy surgery in elderly patients were studied. Eighty ASA patients phys. status I or II of both sexes, aged over 65 yr, weighing 45-70 kg, undergoing laparoscopic cholecystectomy, were randomly divided into parecoxib Na (group P, n=40) and control groups (group C, n=40). The patients in group P were given i.v. parecoxib 40 mg before induction of anesthesia. Ones in group C were given equal volume normal saline instead of parecoxib. Neuropsychol. testing was performed with mini-mental state examination (MMSE) on the preoperative day, 1st and 3rd postoperative day. Venous blood samples were collected before induction of anesthesia (T0), immediately after skin incision (T1), 30 min after CO2 pneumoperitoneum (T2), at the end of operation (T3), the first day of operation (T4), the third day of operation(T5) for determination of serum concentrations of tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) by enzyme linked immunosorbent assay (ELISA). The development of POCD was recorded within 3 days after surgery. Compared with preoperative 24 h [group C (28.2±1.2), group P (28.8±1.0)], the MMSE scores of resting in group C and group P were significantly reduced [(23.0±2.5), (25.0±2.8)] (P<0.05). The MMSE scores of resting in group P were more than those in group C [(25.0±2.8) vs (23.0±2.5)] (P<0.05). The development of POCD within 72 h was less than that in group C (P<0.05). Compared with group C, the serum concentrations of TNFa [(11.8±1.7), (13.8±1.3), (12.3±1.6) ng/L] and IL-6 [(11.8±1.7), (13.8±1.3), (12.3±1.6) ng/L] at T3-T5 were significantly decreased in group P (P<0.05). Compared with T0, the serum concentrations of TNF-α and IL-6 at T3-T5 were increased in two group (P<0.05). Parecoxib Na might decrease the development of POCD in elderly patients undergoing laparoscopic cholecystectomy by inhibition of inflammatory responses.

To discuss the analgesic effect of parecoxib sodium on patients who have undergone subtotal thyroidectomy. The clin. data of 105 cases who had undergone subtotal thyroidectomy in our hospital from Nov. 2014 to Nov. 2015 were collected. The patients were given i.v. injection of parecoxib sodium (40 mg) 15 min before the end of operation (group A). The patients were given i.v. injection of parecoxib sodium (40 mg) immediately after patients returned to the wards (B group). The patients were given i.v. injection of NS (2 mL) solution 15 min before the end of operation (group C). Each group included 35 cases. The visual simulation pain score (VAS score) at 0, 2, 4, 6, 12, and 24 h after returning to the ward, postoperative analgesia satisfaction rate 1 d after operation and the incidence of side effects of the three groups were observed and compared. The VAS scores of A and B groups at each time point were significantly lower than that of C group, with statistically significant difference (P < 0.05). The VAS scores of A group (0, 2 h) were lower than those of B group, with statistically significant difference (P < 0.05). The postoperative occurrence rate of side effects of the three groups had no statistical significance (P > 0.05). Application of parecoxib during or after thyroid subtotal surgery can reduce postoperative pain and increase satisfaction of analgesic effect. Moreover, application of parecoxib is safe and reliable. Giving i.v. injection of parecoxib sodium 15 min before the end of surgery is a better choice.

Background : Isocapnic hyperventilation (IHV) shortens recovery time after inhalation anesthesia by increasing ventilation while maintaining a normal airway carbon dioxide (CO2)-level. One way of performing IHV is to infuse CO2 to the inspiratory limb of a breathing circuit during mech. hyperventilation (HV). In a prospective randomized study, we compared this IHV technique to a standard emergence procedure (control). Methods : Thirty-one adult ASA I-III patients undergoing long-duration (>3 h) sevoflurane anesthesia for major head and neck surgery were included and randomized to IHV-treatment (n = 16) or control (n = 15). IHV was performed at minute ventilation 13.6 ± 4.3 L/min and CO2 delivery, dosed according to a nomogram tested in a pilot study. Time to extubation and eye-opening was recorded. Inspired (FICO2) and expired (FETCO2) CO2 and arterial CO2 levels (PaCO2) were monitored. Cognition was tested preoperatively and at 20, 40 and 60 min after surgery. Results : Time from turning off the vaporizer to extubation was 13.7 ± 2.5 min in the IHV group and 27.4 ± 6.5 min in controls (P < .001). Two minutes after extubation, PaCO2 was 6.2 ± 0.5 and 6.2 ± 0.6 kPa in the IHV and control group resp. In 69% (IHV) vs 53% (controls), post-operative cognition returned to pre-operative values within 40 min after surgery (NS). Incidences of pain and nausea/vomiting did not differ between groups. Conclusions : In this randomized trial comparing an IHV method with a standard weaning procedure, time to extubation was reduced with 50% in the IHV group. The described IHV method can be used to decrease emergence time from inhalation anesthesia.

Results The escape latency was (39.98±8.93) s in Pare group vs (47.15±10.87) s in Sur group (P<0.05); and the space exploration was (24.17±8.52) s vs (20.96±9.58) s (P<0.05). IL-6 expression was (72.86±6.82) in Pare group vs (79.26±4.19) ng·L-1 in Sur group, IL-1β expression was (350.73±28.04) vs (423.35±26.81) ng·L-1 (P<0.05). The horizontal and vertical scores were significantly higher in Pare group than those in the Sur group (P<0.05). The sucrose consumption and sucrose preference were higher and the water consumption was lower in Pare group than those in the Sur group (P<0.05). Conclusion Parecoxib sodium may improve postoperative cognition of elderly rats with depression, which being associated with decrease of IL-6 and IL-1β expression in hippocampus.

[Result]: The postoperative analgesia effect of the advanced analgesia group was significantly better than that of the analgesia pump group (P < 0.05), the range of knee movement was significantly greater than that of the analgesia pump group (P < 0.05), and the number of adverse reactions was significantly less In the analgesic pump group (P < 0.05). Results The data of MAP, SAS and HR in group A and group B were significantly lower than those in group C, and the data of MAP, SAS and HR in group A were significantly lower than those in group B (P<0.05). The data of RSS in group A was significantly lower than that in group B and RSS in group A and group B significantly higher than those in group C from T2 to T4 (P<0.05). The data of VRS in group A and group B were significantly lower than those in group C, and that in group A was significantly lower than group B each time point (P<0.05). Postoperative BCS in group A and group B were significantly higher than those in group C, and that in group A was significantly higher than group B (P<0.05). Drug-induced acute kidney injury (D-AKI) is increasingly common and can extend the hospital length of stay and increase mortality. This study is aimed at analyzing the clin. characteristics of hospitalized patients with D-AKI and the associated risk factors in a multidrug environment. A retrospective study among hospitalized patients was conducted in July 2019 based on the Adverse Drug Events Active Surveillance and Assessment System-2 developed by the authors. Four controls were matched with each case according to the matching criteria. The risk factors for D-AKI were identified by binary multivariate logistic regression. A total of 23,073 patients were hospitalized in July 2019, 21,131 of whom satisfied the inclusion criteria. The independent risk factors for D-AKI consisted of alc. abuse (odds ratio (OR), 2.05; 95% confidence interval (CI), 1.04-4.07), nonsteroidal anti-inflammatory drug (NSAID) use (OR, 2.39; 95% CI, 1.25-4.58), diuretic use (OR, 2.64; 95% CI, 1.42-4.92), prior anemia (OR, 4.10; 95% CI, 1.94-8.67), and prior chronic kidney disease (OR, 2.33; 95% CI, 1.07-5.08). The occurrence of D-AKI in hospitalized patients had significant associations with alc. abuse, combination therapy with NSAIDs or diuretics, and prior anemia or chronic kidney disease. Clinicians should meticulously follow patients with the above characteristics.
