In this study, the authors developed and validated a new, rapid, specific and sensitive ultra-performance liquid chromatog.-tandem mass spectrometric (UPLC-MS/MS) method to simultaneously determine parecoxib sodium (PX) and its active metabolite, valdecoxib (VX), in rat plasma. Plasma samples were prepared by plasma protein precipitation combined with a liquid-liquid extraction method. The separation was carried out on a Kinetex C18 column (2.1 mm × 50 mm, 2.6 μm) with a gradient elution using methanol (A) and a 2 mM ammonium acetate aqueous solution (B). The anal. was performed in less than 3 min with a flow rate of 0.2 mL/min. Ketoprofen was used as an internal standard (IS). Mass spectrometric detection was conducted with a triple quadrupole detector equipped with electrospray ionization in the neg. ion mode (ESI) using multiple reaction monitoring (MRM). The calibration curves were linear over the concentration ranges of 5-4000 ng/mL for PX and 5-2000 ng/mL for VX with all correlation coefficients greater than 0.998. The intra- and inter-day relative standard deviations (RSD) for both analytes were within 15% and the accuracy was within 85-115% at all quality control levels. The mean extraction recoveries for all analytes obtained from three concentrations of QC plasma samples were more than 89.0% efficient. Selectivity, matrix effect, dilution integrity and stability were also validated. The method was successfully used to investigate the pharmacokinetics of PX and VX in rat plasma after i.v. and i.m. administration of PX.

Our objective is to analyze and observe the different administration routes of parecoxib sodium pretreatment on the behavioral improvement of rats with neuropathic pain to provide the preclin. data of parecoxib sodium on neuropathic pain treatment. 30 SD rats were randomly divided into five groups, including model group, sham operation group, intrathecal injection group (IT group), i.p. injection group (IP group), and perineural infiltration group (PI group). The rats in model group and three parecoxib sodium pretreatment groups received spinal nerve ligation (SNL). Heat pain test and 50 % paw mech. withdrawal threshold test (50 % PMWT) were use to assess the responses after parecoxib sodium pretreatment. 50 % PMWT results of right foot in five groups had no statistical difference (P > 0.05); 50 % PMWT results of left and right feet in three parecoxib sodium pretreatment groups were obviously higher than SNL group at different time points, which was statistically different (P < 0.05); in comparison with three pretreatment groups, the data of left foot in IT group were obviously higher than PI group and IP group, and the comparison among three groups had significant difference (P < 0.05). However, the data of right foot had no significant difference among three groups (P > 0.05). Paw thermal withdrawal latency (PTWL) results of left and right feet in five groups had no significant difference before surgery (P > 0.05); after the establishment of neuropathic model, PTWL results in five groups were significantly decreased; however, PTWL results of left and right feet at 3 days after surgery in IT group were significantly higher than the two other pretreatment groups (P < 0.05); PTWL results of left and right feet at 7 and 14 days after surgery had no significant difference. Parecoxib sodium pretreatment can effectively improve the behaviors caused by neuropathic pain, and intrathecal injection is the most effective route of administration.

Methods: 92 Cases of elderly patients with lumbar intervertebral disk protrusion were randomly divided into control group and parecoxib sodium pretreatment group (pretreatment group), 46 cases in each group. The control group was i.v. injected with 2 mL of physiol. saline before surgery, while the pretreatment group was i.v. injected with 40 mg of parecoxib sodium. Both groups were treated with analgesia pump after surgery. The visual analog scale (VAS) scores after 2, 4 and 6 h of surgery, times of patient-controlled i.v. analgesia (PCIA) after 4, 6 and 12 h of surgery, dosage of sufentanil in the analgesia pump after 24 h, and the number of patients with postoperative adverse reactions were recorded in both groups. Results: The VAS scores after 2, 4 and 6 h of surgery and times of PCIA after 4, 6 and 12 h of surgery in the pretreatment group were significantly lower than those in the control group (P < 0.05), and the dosage of sufentanil in the analgesia pump after 24 h in the pretreatment group was higher than that in the control group. The number of patients with nausea, vomiting, hypotension, fever and tachycardia in the pretreatment group was significantly lower than that in the control group (P < 0.05). Conclusion: Parecoxib sodium has effective postoperative analgesia effect on elderly patients with lumbar intervertebral disk protrusion, and can reduce the dosage of medicine in the analgesia pump and reduce adverse reactions.

Objective: To investigate the effect of parecoxib sodium on postoperative analgesia in elderly patients after craniotomy. Methods: 123 elderly patients after craniotomy from Jan, 2011 to Jun, 2014 were selected, and were divided according to random number table into group I, group II, and group III with 40 cases in each group. Patients in group I were i.v. given parecoxib sodium 40 mg 10 min before induction of anesthesia, patients in group II were i.v. given parecoxib sodium 40 mg at the end of the surgery, and patients in group III were not given any analgesic drugs. The visual analog score (VAS), sedation score (Ramsay), β-endorphin level, postoperative adverse reaction and satisfaction of the patients were statistically collected. Results: There were significant differences in VAS between three groups at different time, VASs of group I and group II were both lower than that of group III, and that of group I was lower than that of group II (P < 0.05). There was no significant difference in Ramsay score between three groups at different time (P > 0.05). In group I, serum β-endorphin level had no significant fluctuation in preoperative time, end of operation, and postoperative time. In group II and group III, serum β-endorphin levels at end of the operation and in postoperative time were elevated compared with preoperative time, and were both higher than that of group I in the same period (P < 0.05). There was no serious adverse reaction after operation in group I, II, and III, and there was no statistical difference in the incidence of postoperative complications between three groups (P > 0.05). There were significant differences in satisfaction 24 h after surgery between three groups, the satisfactions of group I and group II were higher than that of group III, and group I was higher than group II (P < 0.05). Conclusion: Preoperative parecoxib sodium administration can improve postoperative analgesia for neurosurgical elderly patients with craniotomy, and has mild adverse reactions, and can inhibit the expression of blood β-endorphin and reduce stress response.

Results After operation 6, 12, 18, 24, 48 h, VAS score of the observation group were lower than the control group, significant difference (P<0.05). After the operation, TNF-alpha, IL-8, IL-6 level, the observation group were lower than those in the control group, significant difference (P<0.05). Conclusion Parecoxib sodium can effectively control pain occurrence degree in lung cancer after radical operation, good analgesic effect.

The detection was carried out with UV detector at 215 nm. The flow rate was 1.0 mL/min. The linear relationship between its major degradation impurity valdecoxib with the concentration of 0.5μg/mL∼5.0μg/mL and the response values of peak area was good, and the correlation coefficients were r=0.9997 and r=0.9992, resp. The average recovery was 100.2%±3.3%(n=9). The method was proved to be accurate, sensitive and reliable, which could be used for determination of the major degradation impurity valdecoxib in parecoxib sodium. The method was used to effectively control the quality of parecoxib.

Objective: To explore the influence of parecoxib sodium pretreatment on analgesia and coagulation function in the patients with chronic hepatitis B cirrhosis undergoing liver spleen operation. Methods: A total of 100 cases of chronic hepatitis B cirrhosis undergoing splenectomy in our hospital from Jan. 2012 to Feb. 2015 were selected and divided into the research group (n = 50) and control group (n = 50). The research group was given 40 mg of parecoxib sodium by i.v. injection at 10 min before anesthesia, while the control group was i.v. injected with 4 mL normal saline at 10 min before anesthesia. Then the pump pressing times during sufentanil self-control vein analgesia, use amount of sufentanil, adverse reactions at postoperative 24 h, and thrombin time (TT), prothrombin time (PT), platelet (PLT) count and fibrinogen(FIB) levels at different postoperative time points were compared between the two group. Results: The pump pressing times within postoperative 24 h during sufentanil self-control vein analgesia in the research group was less than that in the control group, and the difference was statistically significant (t = 5.0, P < 0.001). TT, PT, PLT count and fibrinogen had no statistical differences between the two groups (P > 0.05). Conclusion: Parecoxib sodium can decrease the dosage of analgesic drugs in the patients with chronic hepatitis B cirrhosis undergoing spleen operation, moreover without affecting the coagulant function.

Transarterial chemoembolization (TACE) is the first-line treatment for patients with unresectable liver cancer; however, TACE is associated with postembolization pain. To analyze the risk factors for acute abdominal pain after TACE and establish a predictive model for postembolization pain. From Jan. 2018 to Sept. 2018, all patients with liver cancer who underwent TACE at our hospital were included. General characteristics; clin., imaging, and procedural data; and postembolization pain were analyzed. Postembolization pain was defined as acute moderate-to-severe abdominal pain within 24 h after TACE. Logistic regression and a classification and regression tree were used to develop a predictive model. Receiver operating characteristic curve anal. was used to examine the efficacy of the predictive model. We analyzed 522 patients who underwent a total of 582 TACE procedures. Ninety-seven (16.70%) episodes of severe pain occurred. A predictive model built based on the dataset from classification and regression tree anal. identified known invasion of blood vessels as the strongest predictor of subsequent performance, followed by history of TACE, method of TACE, and history of abdominal pain after TACE. The area under the receiver operating characteristic curve was 0.736 [95%] confidence interval (CI): 0.682-0.789, the sensitivity was 73.2%, the specificity was 65.6%, and the neg. predictive value was 92.4%. Logistic regression produced similar results by identifying age odds ratio (OR) = 0.971; 95%CI: 0.951-0.992; = 0.007, history of TACE (OR = 0.378; 95%CI: 0.189- 0.757; P = 0.007), history of abdominal pain after TACE (OR = 6.288; 95%CI: 2.963- 13.342; P <0.001), tumor size (OR = 1.978; 95%CI: 1.175-3.330; P = 0.01), multiple tumors (OR = 2.164; 95%CI: 1.243-3.769; P = 0.006), invasion of blood vessels (OR = 1.756; 95%CI: 1.045-2.950; P = 0.034), and TACE with drug-eluting beads (DEBTACE) (OR = 2.05; 95%CI: 1.260-3.334; P = 0.004) as independent predictive factors for postembolization pain. Blood vessel invasion, TACE history, TACE with drug-eluting beads, and history of abdominal pain after TACE are predictors of acute moderate-to-severe pain. The predictive model may help medical staff to manage pain.

Objective: To investigate the effects of parecoxib sodium at the different injection time on postoperative analgesia and β-endorphin in patients undergoing resection of cerebral hemisphere glioma. Methods: 96 Cases of patients undergoing the resection of cerebral hemisphere glioma were selected as study subjects and were randomly divided into group A, group B and group C, each with 32 cases. 10 Min before induction of anesthesia, the group A was i.v. injected with parecoxib sodium 40 mg. At the end of operation, the group B was i.v. injected with parecoxib sodium 40 mg. The group C was not given any analgesic drugs. The visual analog scale (VAS) score, Ramsay score, β-endorphin level, the incidence of postoperative adverse reactions and satisfaction were statistically analyzed. Results: The VAS score in group A and group B was lower than that in group C (P < 0.05). 12 And 24 h after operation, the VAS in group A was lower than that in group B, with statistically significant difference (P < 0.05). At the different time points, the Ramsay score had no statistically significant difference among three groups (P > 0.05).
